Deux passagers clandestins découverts dans un avion sont amenés à l'hôpital après avoir subi de graves blessures durant le voyage. Sharon Goodwin reçoit donc la visite du Département de la sécurité intérieure pour obtenir des informations. Cornelius Rhodes, le père du Dr Connor Rhodes, est honoré après avoir fait un don important à l'hôpital pour l’ouverture de l’aile psychiatrique, faisant remonter de sombres secrets de famille. Le grand jour de Dr. Natalie Manning arrive et les bonnes intentions du Dr Will Halstead pour l’aider lors de l’accouchement sont mal interprétées par sa belle-mère Helen.
Pendant ce temps, le Dr Sarah Reese demande de l’aide au Dr Charles pour annoncer une nouvelle déchirante à un jeune garçon, mais aussi annoncer à sa famille que leur autre fils pourrait également être gravement malade.

Connor Rhodes: All right, let's get the X-ray and the FAST scan in here now. Trach set. Got it?

Maggie Lockwood: Got it. I'll bag him.

Connor Rhodes: How's she doing over here?

Will Halstead: Heart rate's 50. Temp is 29 Celsius. Still responding.

Ethan Choi: Breath sounds bilaterally. Right is really muffled.

Connor Rhodes: There's blood throughout his belly. We need to get him up to the OR now. He's gonna need a lap.

Natalie Manning: Wonderful, you know, considering the fact that I can't sleep longer than 15 minutes, and when I do actually get comfortable, I have to pee.

Will Halstead: Fun!

Natalie Manning: Yup.

Maggie Lockwood: Did you try that pillow I gave you?

Natalie Manning: Yes. I added to the 20 other pillows that aren't helping me sleep, but thank you.

Maggie Lockwood: Oh. Sarah's got a kid with a fractured clav in Treatment 1 for you.

Sharon Goodwin: Thanks for coming, Dr. Rhodes. These are agents McManus and Ramirez from Homeland Security.

Ramirez: Dr. Rhodes, you have a female patient, a stowaway?

Connor Rhodes: Yes, Maria Gonzalez.

Ramirez: And she was brought in with a companion?

Connor Rhodes: She was. Miguel, her brother.

Ramirez: Her brother?

McManus: We understand he's unconscious in the ICU.

Connor Rhodes: Mm-hmm.

McManus: We would like to talk to Miss Gonzalez.

Connor Rhodes: Well, I'm afraid that she's barely conscious herself. She's in no condition to be questioned right now.

McManus: Any idea when we'd be able to talk to Miss Gonzalez?

Connor Rhodes: It's too early to say. This woman is a victim. Her father is a narco. He was planning on selling her. Her brother was only trying to save her.

McManus: Sounds like you had extensive conversations with the semi-conscious Miss Gonzalez.

April Sexton: ORs are busy, at least a two-hour wait.

Connor Rhodes: That's too long. We're gonna do the fasciotomies here. Draw up to syringes of ketamine, 70 milligrams apiece, and get me 2 of Versed.

Will Halstead: Whoa, wait. You're not gonna intubate her?

Connor Rhodes: Not necessary. We can do this with deep sedation.

Will Halstead: The ketamine won’t give you much time.

Connor Rhodes: Well, then I will be fast.

Maria Gonzalez: Mi hermano Miguel.

Connor Rhodes: Give her the Versed, follow it up with the ketamine.

April Sexton: On it. BP's 120 over 82. Heart rate's 110. Sat's at 99%.

Connor Rhodes: Okay. Here we go. Okay. I'm going for the posterior compartment now. Keep her still.

Will Halstead: I'm telling you we should be in the OR.

Connor Rhodes: Just suction her out. April, 70 more of ketamine, and then come down here and help hold the leg.

April Sexton: Got it. Second dose is... In.

Connor Rhodes: Okay. Almost... Okay. Okay. Muscle looks viable, bleeds well. That should do it. Wrap her up. Send her up to the ICU.

April Sexton: Got it.

Connor Rhodes: Let me know when she's alert. I need to talk to her. Her brother just died.

Will Halstead: Guy's father writes a big check and he acts like he owns the joint.

Maggie Lockwood: I thought you said you grew up in Chicago.

Sarah Reese: I got Parker's test results back.

Natalie Manning: And?

Sarah Reese: You need to take a look. Here.

Natalie Manning: CT negative. We need to order...

Sarah Reese: An MRI, I know. I already did it. It shows excessive demyelination.

Natalie Manning: Explains the clumsiness, degeneration of motor skills...

Sarah Reese: And the seizure. I don't want to jump to conclusions, Krabbe Disease. Yeah. He's eight, so it's late-onset.

Natalie Manning: Before he becomes confined to a bed and completely vegetative. Poor kid.

Sarah Reese: What do you want to do?

Natalie Manning: Well, I've got to go talk to the par...

Sarah Reese: Dr. Manning? You okay?

Sarah Reese: Dr. Charles? Dr. Manning said you could help me.

Daniel Charles: What's up?

Sarah Reese: I have a patient, an eight-year-old boy, Parker Kindler, and... Uh... You know what, let me back up for a second. Dr. Manning and I were treating Parker, and then she went into labour, and...

Daniel Charles: Natalie's in labour?

Sarah Reese: Mm-hmm.

Daniel Charles: How's she doing?

Sarah Reese: Fine, I suppose. Her water broke right in front of me, but that's not what I came to talk to you about.

Daniel Charles: Okay.

Sarah Reese: Dr. Manning and I just diagnosed Parker with a terminal genetic condition called Krabbe Disease, and now she's in labour, and I have to tell the parents that their eight-year-old child is going to die, and that the same thing might happen to their three-year-old. So I guess I was just wondering... How do... How do I do that?

Daniel Charles: Have a seat.

Sarah Reese: Yeah.

Daniel Charles: In my experience, it's best to be direct and get straight to the point. When people get bad news like this, they tend to get overwhelmed. They might ask you to clarify things or repeat them. You know... They might seem like they have a million questions. They might want to be left alone to begin to process the grief. The most important thing is that they need to know that somebody's there who understands what they're going through. You know...

So really it's your compassion that they need to feel.

Sarah Reese: Compassion.

Daniel Charles: Yeah. And after you tell the parents... They might want you there when they tell the child. They might not. That's up to them.

Sarah Reese: Wait. Tell the child?

Daniel Charles: Oh, yeah. The child actually needs to know.

Sarah Reese: Okay.

Daniel Charles: Look, Sarah, it's the hardest thing that you'll have to do as a doctor, right? And it doesn't get any easier. At least, it hasn't for me. But we are the ones who have been chosen to do this, and I think that there's a certain nobility in that. Okay? Good… You want me to come along?

Sarah Reese: That would be great. Thank you.

Sarah Reese: Given his current symptoms, Parker has anywhere from five to seven years before the progressively degenerative nature of the disease confines him to a bed and eventually leaves him in a vegetative state.

Lisa Kindler: I mean, there must be some kind of treatment.

Sarah Reese: At the moment, no.

Mr. Kindler: You mean there's no cure? There's nothing we can do?

Sarah Reese: Um... That is correct. Krabbe is an extremely rare disease. We can treat his symptoms, but eventually Parker will need to be ventilated to continue living… There is something else. Since the disease is hereditary, we need to determine if Seth carries it as well. Do you know if he's been tested?

Lisa Kindler: Seth? I don't remember.

Sarah Reese: That's okay. I can find out. Do I have permission to check his natal records?

Lisa Kindler: Of course.

Mr. Kindler: What happens if Seth tests positive?

Sarah Reese: Since he has not yet presented symptoms, the hope is we can treat him with stem cells and stop him from developing the disease.

Mr. Kindler: The hope?

ça y est Nathalie est maman. Trop mignon le bébé mais là belle mère est vraiment pas sympa avec Will. Dure journée pour connor... se sentir impuissant comme ça est dur pour un chirurgien. Il sauve la vie de la jeune fille et pouf elle repart pour des raisons politiques. Et avec l'anniv de la mort de sa mère ça fait bien des raisons de craquer.. Terrible le cas de l'enfant condamné mais le Dr Charles arrive à lui expliquer ça en douceur et nous arrache même un sourire au passage
